Clinical Logbook: 

It is a structured record of your patient encounters and clinical reasoning. Enter data soon after patient interaction, using concise and factual language. Follow-up entries must summarise evolution, response to treatment, and complications. and finally, colour-code the reviewed cases by Dr Nizab (🟩 green or 🔵blue as seen on the DIMS webpage. Use blue for highly interesting cases). 

Minimum expected entries per month: Six

Personal Development Plan (PDP):

The Monthly Personal Development Plan (PDP) is designed for short-term, focused improvement in your clinical, communication, and academic skills. It promotes self-directed learning and accountability, with regular mentor feedback. Identify 2–3 specific goals for the upcoming month using the SMART format (Specific, Measurable, Achievable, Relevant, Time-bound). Submit by the 1st Wednesday of each month, to be evaluated by the last Wednesday of the month. One week buffer after evaluation — no new PDP. Use this week for rest, reflection, and feedback integration. Each PDP entry should contain: SMART Objective,  Action Plan (e.g., “Perform 5 ABG interpretations under supervision”), Evidence / Indicator – how progress will be measured, Reflection & Mentor/Dr Nizab's feedback.
